Patient Presentation
- Age: 32 Years
- Gender: Female (Fashion Designer)
- Chief Complaint: The patient was unhappy with the “harmony” of her smile. She felt her teeth looked “short” and “worn out,” and her gum line was uneven. She wanted a smile makeover but was terrified that the final result might look “fake” or “too big” for her face.
Clinical Examination & Diagnosis
- Facial Analysis: The patient had a dynamic smile, but the dental midline did not align with her facial midline.
- Dental Analysis: Generalized attrition (wear) of front teeth was noted, making them look square and aged.
- Gingival Display: Asymmetry in the gum levels (Gingival Zenith) was observed.
- Diagnosis: Disharmonious Smile Arc with dental attrition and gingival asymmetry.
Treatment Objectives
- Restore the ideal length and proportion of the teeth.
- Correct the gum line symmetry.
- Crucial Objective: Allow the patient to visualize and “test drive” the final result before any permanent procedure begins.
The Digital Smile Design (DSD) Workflow
Unlike traditional methods, Dr. Afsana utilized the DSD protocol to plan the smile digitally:
- Step 1: Photo & Video Protocol: High-resolution photographs and videos of the patient were taken from various angles to analyze her facial expressions and lip dynamics.
- Step 2: Digital Planning: Using DSD software, Dr. Afsana drew the ideal tooth shapes and positions over the patient’s digital photo, strictly following the “Golden Proportion” of aesthetics.
- Step 3: The Mock-up (The “Wow” Moment): Based on the digital drawing, a temporary resin model (Mock-up) was placed directly in the patient’s mouth. This allowed the patient to see her new smile in the mirror and take videos before a single tooth was touched.
Final Treatment Execution
Once the patient approved the Mock-up design, the actual treatment was performed:
- Gingivoplasty: Minor laser gum contouring was done to balance the gum heights as per the digital guide.
- Preparation: Minimal preparation of the teeth was done through the mock-up (aptitude-guided preparation) to preserve maximum enamel.
- Restoration: Lithium Disilicate (E-max) Veneers were fabricated that exactly replicated the approved design.
Results and Outcome
- Predictability: The final result matched the digital simulation 100%.
- Aesthetics: The teeth appeared longer, youthful, and symmetrical, perfectly complementing her face shape.
- Patient Satisfaction: The patient was emotional upon seeing the result, stating, “It looks exactly how I imagined it during the trial.”
Conclusion
Digital Smile Design eliminates the guesswork from cosmetic dentistry. Under the artistic vision of Dr. Afsana Haque Joty at Tech Dental, patients don’t just hope for a good result; they co-design their smile and see the future outcome beforehand, ensuring total satisfaction.
