Patient Presentation
- Age: 24 Years (Estimated)
- Gender: Female
- Chief Complaint: The patient presented with a severely broken upper front tooth. She reported dissatisfaction with her smile due to the visible gap/broken structure and expressed concern about saving the tooth rather than extracting it.
Clinical Examination & Diagnosis
Upon clinical examination, the following were noted:
- Fractured Crown: The upper right central incisor had lost a significant portion of its coronal structure, likely due to trauma or severe decay.
- Pulp Involvement: The fracture extended deep into the tooth structure, compromising the pulp tissue, necessitating endodontic intervention.
- Aesthetic Compromise: The broken tooth created a negative space in the smile line, affecting the patient’s self-confidence.
- Diagnosis: Complicated Crown Fracture with Pulp involvement requiring endodontic therapy and prosthetic rehabilitation.
Treatment Objectives
- Remove infected/damaged pulp tissue to prevent infection and pain.
- Preserve the natural tooth root structure.
- Restore the anatomical form and function of the tooth.
- Achieve a natural aesthetic blend with the adjacent teeth.
Treatment Plan: Root Canal Treatment (RCT) & Crown
To save the natural tooth and restore its look, a two-phase plan was adopted:
- Root Canal Treatment (RCT): To clean and seal the inside of the tooth.
- Dental Crown (Cap): To cover the remaining tooth structure, providing strength and aesthetics.
Clinical Procedure
Cementation: The final custom-made crown (Cap) was permanently cemented onto the prepared tooth structure.
Endodontic Phase: Access was gained to the pulp chamber. The canals were thoroughly cleaned, shaped, and sterilized. They were then sealed with a biocompatible material (Gutta-percha) to prevent future bacterial entry.
Post & Core Build-up: Due to extensive structure loss, a core build-up was performed to create a solid foundation for the crown.
Crown Preparation: The tooth was shaped to receive the dental cap.
Shade Selection: A shade matching the patient’s adjacent natural teeth was carefully selected to ensure the cap looked undetectable.
Results and Outcome
The post-operative results (as seen in the second image) demonstrate a successful rehabilitation:
- Aesthetics: The new crown perfectly mimics the shape, size, and color of the natural teeth. The smile is full and confident.
- Function: The patient can now bite and chew normally without fear of breaking the tooth.
- Gum Health: The gum line around the cap appears healthy and pink, indicating good tissue response to the restoration.
Conclusion
This case highlights the importance of Root Canal Treatment combined with a Dental Cap in saving severely damaged teeth.
