1. Patient Presentation
- Age: 38 Years
- Gender: Male
- Chief Complaint: The patient presented with a missing lower right back tooth (First Molar). He complained of difficulty in chewing food on that side and was concerned about the adjacent teeth shifting into the empty space.
2. Clinical Examination & Diagnosis
Upon clinical examination and CBCT (3D X-ray) analysis, the following were noted:
- Missing Tooth: Tooth 46 (Lower Right First Molar) was missing (extracted 2 years ago due to decay).
- Bone Density: Fortunately, the patient had sufficient alveolar bone height and width to support an implant without extensive bone grafting.
- Adjacent Teeth: The neighboring teeth were healthy but showed early signs of tilting toward the gap.
- Diagnosis: Partial Edentulism (Missing single posterior tooth).
3. Treatment Objectives
- Restore full chewing efficiency (Masticatory function).
- Prevent bone resorption (bone loss) in the jaw.
- Preserve the structure of adjacent healthy teeth (avoiding a dental bridge which requires cutting neighboring teeth).
- Provide a long-term, fixed solution.
4. Treatment Plan: Endosteal Dental Implant
The patient was educated about the difference between a Dental Bridge and an Implant. He chose the Dental Implant because it is a standalone restoration that does not damage other teeth and mimics a natural tooth root.
5. Clinical Procedure
The treatment was carried out in two phases:
Phase 1: Surgical Placement
- Anesthesia: The procedure was performed under local anesthesia to ensure a painless experience.
- Implant Placement: A titanium implant fixture (artificial root) was surgically placed into the jawbone with precision.
- Osseointegration: The site was allowed to heal for 3 months to allow the implant to fuse biologically with the bone.
Phase 2: Prosthetic Restoration (After 3 Months)
- Abutment Placement: Once the implant was fully integrated, a connecting piece called an abutment was attached.
