Chief Complaint: The patient reported dissatisfaction with the “shortened” and “broken” appearance of his front teeth. He also complained of mild sensitivity to cold liquids and felt that his smile looked aged.
Clinical Examination & Diagnosis

Upon clinical examination, the following were noted:
Incisal Attrition: Significant wear on the biting edges (incisal edges) of both upper and lower anterior teeth, likely due to a history of bruxism (tooth grinding).
Chipping: Irregular fractures on the enamel of teeth 11 and 21 (upper central incisors).
Dentin Exposure: The yellowish hue on the lower teeth indicated that the protective enamel had worn away, exposing the underlying dentin.
Diagnosis: Pathological tooth wear and enamel chipping leading to compromised aesthetics and function.
Treatment Objectives
Restore the lost tooth structure to a natural length.
Improve the “smile line” and overall facial aesthetics.
Protect the remaining tooth structure from further wear and sensitivity.
Provide a cost-effective, minimally invasive solution.
Treatment Plan: Direct Composite Veneers
While porcelain veneers were discussed, the patient opted for Direct Composite Veneers due to the immediate results and the “no-prep” or “minimal-prep” nature of the procedure, which preserves more of the natural tooth.
Clinical Procedure
Shade Selection: Using a VITA shade guide, a combination of body and enamel shades was selected to mimic the natural translucency of the patient’s teeth.
Preparation: Minimal roughening of the enamel surface was performed to enhance the bonding strength. No significant tooth reduction was required.
Bonding: A total-etch technique followed by the application of a high-strength bonding agent was used.
Layering: The composite resin was applied using a layering technique. This involves building up the “dentin” core first and then overlaying it with “enamel” resin to create a lifelike, three-dimensional effect.
Finishing & Polishing: The restorations were shaped using fine-grit diamonds and polished to a high luster using multi-step polishing discs and paste to ensure a smooth surface that resists plaque and staining.
Results and Outcome
The post-operative results (as seen in the second image) show a significant transformation:
Aesthetics: The teeth now have a youthful length and a symmetrical, harmonious appearance.
Function: The incisal edges were restored to proper alignment, improving the patient’s bite.
Patient Satisfaction: The patient expressed high satisfaction with the immediate results and the painless nature of the treatment.
